BCBS Silver PPO + GAP Coverage
Lower premium + GAP protection
Your BCBS Silver plan works like a normal PPO with deductibles and copays.
GAP helps reimburse certain higher-cost medical events.

Choose Silver + GAP if:

Choose Platinum if:

β’ You frequently use medical services
β’ You prefer predictable copays and costs

How GAP Coverage Works
GAP is a secondary coverage that helps reduce big medical expenses.
You always use your BCBS Alabama plan first.
What GAP does cover:
Inpatient hospital stays (room & board, surgery)
Outpatient surgery centers
Major diagnostics: MRI, CT, scans
Often emergency room services (depending on plan)
Helps pay a portion of deductible/coinsurance bills
What GAP does NOT cover:
Regular office visit copays alone
Services not covered by BCBS
Out-of-network services (beyond BCBS allowance)
Every BCBS cost β only applies to certain categories
1. Use BCBS normally (doctor/hospital files claim).
2. BCBS sends you an EOB explaining what you owe.
3. You upload the EOB to the GAP carrier (or follow employer instructions).
4. GAP sends you a reimbursement check or direct deposit.
GAP is designed to reduce the financial burden of major medical events.
Dental Coverage
Covers cleanings, x-rays, fillings, root canals, crowns, and other dental needs.
Your dental choice:
Dental plans usually cover preventive services at 100%, plus benefits for fillings,
crowns, and major services depending on the plan.
Vision Coverage
Helps pay for eye exams, glasses, frames, and contact lenses.
Your vision choice:
Vision insurance is helpful if you wear glasses or contact lenses, or prefer annual eye exams.
Life Insurance & Beneficiaries
Life insurance provides financial support to someone you choose (your beneficiary).
Voluntary life coverage:
Make sure your beneficiaries are up to date.
Disability Coverage (Income Protection)
Provides income if you cannot work due to an illness, injury, or childbirth recovery.
Your disability choice:
Accident, Hospital, Critical Illness
These plans pay cash directly to you if certain events occur.
Your supplemental choice:

β’ Hospital β pays for inpatient stays
β’ Critical Illness β pays a lump sum for major diagnoses
HSA / FSA
Pre-tax accounts can help with medical, dental, and childcare expenses.
Contribution level:
HSA = yours forever. Rolls over.
FSA = βuse it or lose it,β with limited rollover (if employer allows).
